- Fall Survey
- Mary E. Phillips Scholarship Fund Application Please complete and submit to your Parent Education Instructor.
- Mary E. Phillips Scholarship Fund Donation Form This form explains the function of our Parent Advisory Council and the scholarship fund it administers.
- Parent Education Class Calendar To allow the presenter to ensure there are enough chairs and handouts, please use the email link or phone number to register with the class Instructor. Classes requiring phone-in registration should be phoned in by the parent.
- Field Trip Permission Form In case you either lost, or didn't receive one, please complete and turn in to your Field Trip Coordinator in advance of the date.
- Field Trip Notification Form for the Field Trip Coordinator to complete and submit in advance of the trip to the Parent Education Instructor.
- Child Release Form Whether you use this form or a handwritten note, should you want your child to go home from preschool with someone not on your list on file, it is REQUIRED that you make that request in writing. Make sure that you sign and date the note/form.
- Child Registration Form to place on file in the preschool.
- Evaluations See your group chair or parent instructor for evaluation timelines.
